"Road to Bariatric Surgery"
Different insurance companies have different criteria for you to qualify or "be approved for bariatric surgery" (Gastric bypass or Lap Band). We highlight here the "Road to Bariatric Surgery" usually followed. Your ROAD may differ slightly, based on your insurance coverage.
1. Generally, you will want to attend a FREE, NO-OBLIGATION, GET ACQUAINTED seminar on bariatric surgery, regularly presented at our centers by one of Bariatric Surgeon. During this 1+ hour seminar, the risks, benefits, and lifestyle requirements of all types of bariatric surgery will be presented. Ample time is provided for all questions to be answered by the surgeon.
2. Once you've attended a SEMINAR, you can call to schedule a CONSULT with the surgeon. (in some locations, consults can be scheduled on same day of seminar to prevent two trips from being made). These are usually offered at least once or twice a month at most centers. You can schedule an appointment through this website or by calling the Program or Surgeon Office, as material will be mailed to you in advance, that will require your completion before your consult with the surgeon.
3. Once you have had your CONSULT with a surgeon, he will make a decision with you whether you are a candidate for bariatric surgery AND what type of bariatric surgery would be right for you.
4. Your insurance company will be contacted and the surgeon's office will prepare and submit the MEDICAL NECESSITY case, requesting their written approval for your surgery.
5. The Program or Surgeon's office will call you once they have heard from your insurance. Frequently, insurance companies will request additional information, which you will need to provide. What is needed will be explained to you, and it will be your responsibility to gather the documentation and return it to your surgeon's office.
6. If you are APPROVED for surgery, the Surgeon's Office will call and schedule your surgery date and the medical clearance tests which you must complete.
ALL PATIENTS SCHEDULED FOR SURGERY MUST HAVE FIRST COMPLETED, AND THE SURGEON'S OFFICE MUST HAVE DOCUMENTATION ON:
Nutritional consult and clearance provided by the Program Registered Dietitian;
Psychological evaluation and clearance provided by a licensed Psychologist or Psychiatrist.
Attendance and active participation in the Pre-Admission Class (PAC) conducted by Registered Dietitian, f required by surgeon
All other medical or other clearances requested by Surgeon
7. After surgery, be sure to keep all your follow-up appointments with your surgeon, and attend monthly support groups.
DO YOU ACCEPT MY INSURANCE?
Chances are we do. The hospitals and surgeons with which we work accept most insurances, and are in network for most insurance. Medicare is accepted only at the Louisville location. Medicaid is not accepted at any location.
IF YOUR INSURANCE DOESN'T COVER LAP BAND OR GASTRIC BYPASS...
If your insurance does not provide coverage for bariatric surgery, reasonable self pay prices ($12,500 for lap band total cost) are available. Contact us for more information AND INFORMATION ON FINANCING OPTIONS AVAILABLE TO YOU.
