Laparoscopic Adjustable Gastric Band
(Lap Band)
Pictured on the left, is the band device that is placed in the stomach during the operation.
The Lap Band Surgery is performed by placing the band around the topmost portion of the stomach, as pictured at the right, above. The band is secured by connecting the stomach to itself over the top of the band. The band has an adjustment tubing which is connected to a port that is secured to the muscles of the abdominal wall underneath the skin. The band has a balloon which makes the opening that food has to pass through either smaller or larger (thus, the term adjustable) by injecting or withdrawing saline through the port. The gastric pouch above the band is about one ounce (30cc) in size and fullness or satiety is reached as this pouch is filled, making it possible to be satisfied with small food portions.
The Lap Band is designed to be placed laparoscopically, which is done by making five incisions.
A major feature of the Lap Band, manufactured by Allergan /Inamed, is that it is adjustable, which allows it to potentially become the "ideal" size for each patient. Other purely restrictive operations (as the Lap Band is) include the Vertical Banded Gastroplasty, which use has been largely discontinued in the United States because of poor weight loss outcome studies. The VBG was not adjustable, though, as the Lap Band is.
FILLS
Patients having the Lap Band operation will need to visit their surgeon regularly for "fills," which bear an additional cost each time. The adjustment or "fill" is completed by accessing the port with a needle and injecting or withdrawing saline. This can be usually be done in the doctor's office, but may from time to time require x-ray guidance.
LAP BAND IS NOT FOR EVERYONE
About 10 percent of the bariatric operations performed in the United States are Lap Bands. However, this procedure is not for everyone.
As noted by the manufacturer of The Lap Band,
Some surgeons say patients with a "sweet tooth" will not do well with the LAP-BAND
If you eat a lot of sweet foods, your surgeon may advise you that this is not the best bariatric surgery for you. The same is true if you often drink milkshakes or other high-calorie liquids.

As with any bariatric surgery or any weight loss program you may begin, YOUR MOTIVATION IS KEY.
Your surgeon will not do the operation unless he or she knows you understand the problems your excess weight is causing. Also, your surgeon will make sure you know you have responsibilities. These include learning and adopting anew eating patterns and a new lifestyle. If you are ready to take an active part in reducing your weight, your surgeon will consider the treatment. First, though, your surgeon will want to make sure you know about the advantages, disadvantages, and risks involved.
