The gastric bypass surgery (GBP) can be performed either open, through a single larger incision, or laparoscopically, through six small incisions. Gastric bypass is both a restrictive and malabsorptive procedure. It is restrictive because a small gastric "Puch" is formed, which will hold one to two ounces of food. The pouch is formed by applying surgical staples. These cut the pouch away from the rest of the stomach, which becomes the Bypassed Portion of the Stomach. After surgery, food will never enter the Bypassed Stomach.

During the operation, the intestine is divided and partially bypassed making this a malabsorptive operation, as well. The gastric, or Roux-Limb, carries food out of the Pouch and is about 3 feet (or 100-150 cm) long.

The biliopancreatic limb, or Duodenum, carries digestive juices from the liver, pancreas, and Bypassed Stomach. No digestion occurs here because the food does not mix with the digestive juices. The food and digestive juices meet in the Common Limb, which is about 15 feet long. The Common Limb starts where the duodenum and Roux-Limbs are joined. Contents pass into the Colon.

This surgery has been performed for more than 20 years, and is considered the Gold Standard of Bariatric Surgery. More than 75 percent of the bariatric surgeries done in the United States each year are either open or laparoscopic gastric bypass operations. 15-year follow-up studies indicate patients have maintained a 60-70% loss of excess body weight.
